Visual confirmation - no codes showing

I have installed ACP 208 which went without a problem. The issue I have is that the visua; confirmation code does not show up at all. There is an empty white space.

Any thoughts ?

Perfect - did the job. I did actually re-upload the other day in binary, however this did not work. the delete and reupload did the trick.
